We perform reviews of patients’ medication regimens at a frequency consistent with current regulations for the setting in which care is provided. We ensure that the patients’ therapies are appropriate given the patients’ variables (proper dosage, drug interactions, proper route and time of administration). Furthermore, we review medication regimens in the interim for new admissions to ensure a smooth transition of care. At the conclusion of each review, we provide patient specific reports with any relevant recommendations to ensure optimal outcomes.
